Cal needs a physical check up, but he's been too busy to go into the clinic for it.  He heard about a new service though, you schedule your checkup and then you are mailed a tiny doctor who will complete the physical and then you just ship him back.
 
The tiny doctor starts checking his balance, taking some measurements, weighing him (the doctor explains that Cal just has to step on him, he is pliable and by measuring the compression force they can calculate his weight.)
 
He also has Cal breath in and out while the tiny doctor listens.  He then checks his nose and mouth.
 
Next he has Cal put him in his mouth to check it, based on his file the tiny doctor asks Cal to swallow him to check inside him.  After struggling for a while Cal manages to cough the tiny doctor back up into the sink.
 
The tiny doctor wants to run a few more tests but Cal has to run so he puts the tiny doctor in his shoe for safe keeping.  But Cal forgets the doctor in his shoe and after a bit comes back and puts the shoe on unaware the tiny doctor is squished under his sock and stuck in his shoe.
